Emergency Department Care Coordination

Emergency Department Care Coordination Program integrates community & home-based Behavioral Health Care Coordination services through a partnership with Atlantic General Hospital, Berlin, MD.  This program facilitates self-care, appropriate uses of behavioral health services and works to reduce hospitalizations due to overdose through improving communication and coordination between the patient, caregiver, AGH, physicians and other medical providers.  Care Coordination staff assist with obtaining Narcan training, identifying and linking patients with resources available in the community and follow-up regarding treatment & appointment coordination.
