Coordination of Community Services (CCS)
The Worcester County Health Department is licensed to provide Coordination of Community Services to individuals with intellectual/developmental disabilities and their families. Coordinators assist eligible individuals and their families in gaining access to resources in their community, planning for their future,education on transition services and waivers that could be available to them as well as, accessing needed services and supports.
Eligibility is determined at a regional level by the Developmental Disabilities Administration (DDA). To become eligible for services from the DDA, individuals must have a disability which keeps them from handling daily activities independently. A developmental disability is a physical or mental impairment (other than a sole diagnosis of mental illness) or it can be a combination of mental and physical impairments that:
- continue lifelong
- onset before the age of 22 years old
- requires support or assistance in order to live independently
- requires support of a team to help with accessing and obtainment of services and treatment
