What is Emergency Department Care Coordination (EDCC)?
Emergency Department Care Coordination is a partnership with TidalHealth Atlantic that:
- Integrates community & home-based Behavioral Health Care Services
- Assists with communication between those involved with care planning
- Assists with identifying and linking individuals with resources available in the community
- Provides follow-up regarding treatments and appointments
Eligibility:
- Risk of hospitalization due to unmet behavioral health needs
- Experienced or is at risk of an overdose
- In need of assistance with finding treatment providers
- Substance misuse is increasing the need for an emergency department visit
Recovery Options (Services for adults and adolescents)
Outpatient Substance Use Services:
- Intakes available by appointment (call location for times)
- Intensive Outpatient
- Individual and Group Counseling
- Medication Assisted Treatment (Vivitrol and Buprenorphine)
- Specialty Program for Pregnant and Postpartum Women
- Peer Support
Outpatient Mental Health Services:
- Intakes available by appointment or walk-in (Call location for times)
- Dual Diagnosis Group
- Individual Mental Health Counseling
- Psychiatry
- Family Therapy
Assistance With:
- Referrals to Inpatient Treatment
- Naloxone (Narcan) Training
- Insurance Needs
- Housing Resources
