Mini Grant Applications FY 25

Below are all available Mini Grants for Fiscal Year 2025. To apply, please download and complete the fillable PDF application for the grant you wish to apply, then email the completed application to the listed recipient. If you need a paper copy, please call 410-632-1100 ext. 1119. Applications for ALL mini grants must be returned to the health department by August 2, 2024 no later than 4:30 p.m. A review team will evaluate the applications, and awards will be made after August 23, 2024. Advancing Health Equity Related to Tobacco

Description: The WorNew Funding OpportunityThe Worcester County Health Department (WCHD) is requesting proposals to address tobacco use disparities as a result of receiving funds from the State of Maryland to advance health equity by identifying and eliminating tobacco, diabetes, and chronic disease related health inequities and disparities. Community-based organizations, churches, private entities, non-profits, and workplaces are encouraged to apply. Organizations can request up to $12,000 and awards will depend on the number and quality of applications received. A total of $24,000 is available to support these efforts. Address Community Tobacco Cessation and Enforcement Projects

New Funding Opportunity: The Worcester County Health Department (WCHD) is requesting proposals to address tobacco control and enforcement as a result of receiving cigarette restitution funds through the State of Maryland. Community-based organizations, churches, private entities, non-profits, and workplaces are encouraged to apply. Organizations can request up to $4,000 for community-based activities and $6,000 for tobacco retailer education. Awards will depend on the number and quality of applications received.

Teen Health Education

The Worcester County Health Department requests mini-grant proposals from community-based organizations, workplaces, churches, or other interested organizations for youth teen pregnancy and sexually transmitted infection (STI) prevention education: Promoting Health Among Teens-Comprehensive education (PHAT-C). Description: The Worcester County Health Department requests mini-grant proposals from community-based organizations, workplaces, churches, or other interested organizations for youth teen pregnancy and sexually transmitted infection (STI) prevention education: Promoting Health Among Teens-Comprehensive education (PHAT-C). To be eligible for up to $4,500 in grant funding, your program must be an organization that serves young people in Worcester County. Funded organizations will be expected to deliver the PHAT-C education program to a minimum of 15-20 Worcester County youth ages 12-19.

True You Maryland Teen Health Community Education Projects

  Description: The Worcester County Health Department requests mini-grant proposals from community-based organizations, workplaces, churches, or other interested organizations for youth teen pregnancy and sexually transmitted infection (STI) prevention education: True You Maryland Teen Health Community Education program. To be eligible for up to $5,000 in grant funding, your program must be an organization that serves young people in Worcester County. Funded organizations will be expected to deliver the True You Maryland Teen Health Community education program to a minimum of 40-50 Worcester County youth ages 14-19.

Support Community Gardens

WCHD  is requesting proposals from community gardens located in Berlin, Pocomoke or Snow Hill to assist with an Alzheimer’s Disease and Associated Dementias project. Funded gardens must commit to assisting with Alzheimer’s Disease and Associated Dementias prevention education, sustaining the garden and providing produce to community residents.Organizations can request up to $2500 and awards will depend on the number and quality of applications received. A total of $5,000 is available to support these efforts.

Support Cognitive Health Screenings

The Worcester County Health Department is requesting proposals from health care provider practices located within the county to assist with these efforts. A total of $16,000 is available to support health care practice initiatives.

Description: The Worcester County Health Department has been awarded funds from the Maryland Department of Health to address cognitive health and increase public awareness about Alzheimer’s Disease and Related Dementias (ADRD) and how to reduce the risk of developing. These funds are also to be used to increase cognitive health screenings. Funds are available to support these efforts through strengthening/establishing cognitive health screening protocols in local health care provider practices; increasing the number of cognitive health screenings completed; providing patients with resources and educational materials and linkage to appropriate programs and/or services; and encouraging providers to participate in training programs that focus on Alzheimer’s and associated dementias.

Adolescent-Focused Health Clinics for True You Maryland Teen Health Education Projects

Description: The Worcester County Health Department requests mini-grant proposals from adolescent-focused health clinics for teen pregnancy and sexually transmitted infection (STI) prevention education: True You Maryland Teen Health Education program. To be eligible for up to $11,000 in grant funding, your program must be a health clinic that serves young people in Worcester County. Funded organizations will be expected to deliver the True You Maryland Teen Health education program to a minimum of 50 Worcester County youth ages 14-19.
