altMaryland Access Point (MAP) is a "one stop shop" for Worcester County residents to access information and services to help aging and vulnerable adults remain in the community. Worcester County MAP consists of a multi-agency platform consisting of Worcester County Health Department, Worcester County Department of Social Services and Worcester County Commission on Aging. The Adult Services programs of these agencies work cooperatively to help aging and vulnerable adults navigate their resource and support options.

Hours: 8:00am- 4:30pm

Eligibility: Any adult 18 years or older (Worcester County residents), regardless of income

Service (s) 

- Single point of entry to Adult Service programs provided by Worcester County Health Dept., Department of Social Services, and Commission on Aging.

-Provides information, assistance and referral to aging, vulnerable, or functionally disabled adults, and their caregivers

-Options Counseling- planning and coordination for long-term care services and planning for future needs.

-Education, system navigation & linkage to community resources. 

-Application/enrollment assistance to community programs & services.

Adult Evaluation and Review Services (AERS)

Hours: 8:00am- 4:30pm

Eligibility: Any Worcester County resident (aging or functionally disabled adult) who is at risk of placement in a long-term care facility. 

Service (s): 

-An AERS Nurse or Social Worker provide a comprehensive assessment in the client's home or current location

-Identify community services available to help residents remain in the community, or in the least restrictive environment

-Develop an individualized plan of care to address the client's identified needs.

-Interpretation services are available
